The Woodbridge Medical Centre Family Health Team (WMC FHT) is a collaborative primary health care team composed of a Dietitian, Family Physicians, Family Practice Nurses, Nurse Practitioner, Pharmacist, Psychotherapists, Social Workers and other Professionals who work together as an interdisciplinary team, to provide patient-led primary health care services and programs.

SOCIAL WORK IS TRANSITIONING ITS ACCESS TO SUPPORTS FROM WALK IN ON TUESDAYS (1PM-6PM) TO SCHEDULED SINGLE SESSIONS APPOINTMENTS.  AS OF TUESDAY, JULY 1ST WALK IN WILL NO LONGER BE AVAILABLE.  See below for more information.

Monday-­Thursday: 5:00 pm to 9:00 pm

In Person Walk-­In Hours:

Monday-­Thursday: 9:00 am to 9:00 pm

Friday: 9:00am to 5:00 pm
Saturday: 9:00 am to 3:00 pm
Sunday: 9:00 am to 1:00 pm
