Welcome to Woodbridge Medical Centre Family Health Team (WMC FHT) is a collaborative primary health care team composed of a Dietitian, Family Physicians, Family Practice Nurses, Nurse Practitioner, Pharmacist, Psychotherapists, Social Workers and other Professionals who work together as an interdisciplinary team, to provide patient-led primary health care services and programs.
