Dear Patient,
It is with a heavy heart that I announce my departure from the Woodbridge Medical Centre Family Health Team (WMC FHT). As you already know, I have been on maternity leave from my practice with Dr. Ashley Esteves as my locum physician. After much consideration and reflection, I have decided to leave my practice to focus more on my family and young children. This has been a challenging decision, as I greatly value the privilege, I’ve had in providing you with care for the past 5 years. I thank you for your trust and for the relationships we made.

The clinic is actively working on a replacement for me and will provide you with more information once available. From now until September 30, 2024, you will continue to receive care primarily from Dr. Ashley Esteves, my maternity substitute locum physician. As of October 1, 2024, my colleagues at Woodbridge Medical Centre Family Health Team will continue to provide care to you until a suitable replacement is found to take over my current practice.

Your medical records will remain at the clinic and you and your next physician will have full access to them with your consent. If you choose to leave the clinic, a copy of your chart will be made available to you according to the current regulations and fees.
